Qualifications/Basic Job Requirements:

  • Bachelor’s Degree in a health-related field and minimum of two years addiction treatment experience or an equivalent combination of education, training and experience.
  • Demonstrated clinical competency in assessments and admissions.
  • Excellent interpersonal skills.
  • Adequate written skills to accurately complete required documentation within the time frames prescribed.
  • Demonstrated clinical competency in assessments and admissions.
  • Complete Iowa DHS Dependent Adult & Child Abuse Mandatory Reporter training within 90 days of start date.
  • Proficient working with Microsoft Office products.
  • Bi-lingual skills (preferred).

Essential Responsibilities:

  1. Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.
  2. Handle all First Contact calls and assess telephonically to determine appropriateness for treatment, level of care, and special needs.
  3. Schedule assessment interviews with potential clients and their significant others.
  4. Schedule and conduct intake interviews, follow up interviews (six- month point), and discharge interviews for grant participants.
  5. Ensure client health records are maintained consistent with rules and regulations, including accurate and timely data entry of grant data into IBHRS and SPARS systems.
  6. Formulate initial diagnostic impression and appropriate recommendations for level of care placement.
  7. Provide care coordination/case management services to assigned clients.
  8. Utilize intervention skills as required to place the client in the appropriate level of treatment services.
  9. Maintain knowledge of community and recovery resources to make appropriate interagency referrals as required.
  10. Provide networking, linkage, and follow-up with referral sources.
  11. Distribute and maintain accurate records for the recovery support services grant participants requests (e.g., bus passes/gas cards, etc.).
  12. Verify insurance if needed.
  13. Have Client Accounts Representative meet with the client if needed to discuss co-pay and get down payment when appropriate.
  14. Maintain communications with all staff (e.g., assessments, admissions).
  15. Notify appropriate staff of client status during admissions process.
  16. Serve as a member of the Access Team and participate in all team meetings and activities.
  17. Understand and comply with all principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.
  18. Perform all responsibilities in compliance with the mission, vision, values and expectations of Rosecrance.
  19. Deliver exceptional customer service consistently to every customer.
  20. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
  21. Assume other related responsibilities as assigned by management.

Job Type: Full-time

Pay: Based on education, experience and credentials

Schedule: 8 hour shifts

  • Monday-Friday, 8AM-5PM
