About the RoleWe are looking for an experienced Lead Social Care Specialist to join our urgent mission! In this role you will lead on the day to day of our social care programming, assist in the implementation of efficient workflows, and manage your own caseload of low to high acuity cases. This is a very hands-on, leadership position and requires knowledge of people management, case management, social care benefits, or behavioral health. In this role, you will become a trusted subject matter expert to your team. 

The right candidate is comfortable with leadership and has a background in Social Work, Public Health, Behavioral Health, or a similar field. 

Qualifications:

  • Create & foster an inspiring team environment with an open communication culture.
  • Act as a subject matter expert on complex case management and/or behavioral health.
  • Provide mentorship and a coaching in an empathetic manner.
  • Lead regularly scheduled team meetings and convey information/changes that are coming from management to the team.
  • Manage your own caseload of moderate to complex cases through communication with patients and caregivers, documentation in EHR systems, data entry, maintenance and reconciliation.
  • Ensure delivery on goals and metrics by proactively managing the day to day of your team and tracking metrics.
  • Collaborate with leadership to establish and/or update workflows to ensure the highest levels of performance and customer satisfaction.
  • Ensure that follow up is completed with both the client and referral entity to ensure the completion of services.
  • Work collaboratively with our Social Care Manager to develop outreach strategies and expand patient knowledge of available resources/services.
  • Collaborate across partners to identify community resources to assist patients 
  • Leverage your in-depth knowledge of behavioral health or government benefits and all available services and stay informed about policy and procedure changes.
  • Complete statistical data, reporting, and/or required paperwork in a timely manner
  • Performs other duties as assigned by leadership.

Requirements:

  • Bachelor’s or Master’s degree in Psychology, Counseling, Social Work, or related field or equivalent work experience.
  • Licensed clinician (LSW, LPC, LCSW, LCPC, LCP) preferred.
  • 3 years of relevant experience in a hospital or social services role. 
  • 1 years of leadership experience. 
  • Bilingual (Spanish) highly preferred
  • Experience with housing, transportation, food insecurity, or other social benefits enrollment and eligibility would be a major plus. 
  • Comfortable communicating in Spanish (ideal but not required).
  • Good judgment in maintaining responsibilities of the position in advocating for the best interest of clients served.
  • Ability to work effectively with diverse individuals and groups.
  • Strong interpersonal and social skills with an ability to collaborate with a variety of individuals from a wide range of professional and personal backgrounds.
  • Pass a pre-employment drug screen and random drug screens and maintain emotional control, and professional composure at all times.
  • Must have access to a vehicle, proof of insurance and valid driver’s license to perform essential job functions. 
  • Must have intermediate to advanced computer skills.
  • Strong oral and written communication skills.
