About the Role

The Associate Social Care Manager is a pivotal role supporting the Social Care Manager in the strategic oversight and delivery of social services for our programming. In close collaboration with company leadership, the ASCM will assist in managing the day-to-day operations of Social Care Specialists, ensuring the delivery of high-quality care to underserved and vulnerable populations.

With a strong foundation in empathy and cultural competence, the ASCM must connect with and understand the unique circumstances and challenges faced by the populations served. The role involves providing subject matter expertise, team support, and guidance, contributing significantly to the team's success. Ideally, we are looking for a candidate who has experience in Social Work, Population Health, Public Health, or Health Care Quality Measures.

Responsibilities:

  1. Subject Matter Expertise: Act as a subject matter expert, providing guidance and support to the Social Care Specialists, ensuring effective social service delivery workflows.
  2. Team Assistance: Assist the SCM in managing the day-to-day activities of the Specialists, fostering a collaborative and high-performance culture within the team.
  3. Team Leadership Support: Aid in supervising Specialists, promoting professional development, intuitive problem-solving, and skills enhancement within the social care field.
  4. Quality Assurance: Collaborate with the care management team to maintain high-quality care standards, monitoring neighborhood network and social service hub performance.
  5. Training and Development: Provide ongoing training to Specialists, ensuring adherence to best practices, accurate documentation, and efficient processes.
  6. Stakeholder Engagement: Assist in building and nurturing impactful relationships with hospital and health plan leadership and management, fostering collaboration and growth opportunities.
  7. Human Resources Support: Participate in the hiring process and annual performance reviews, ensuring the team is adequately staffed and meets performance targets.
  8. Healthcare and Technology Integration: Navigate the intersection of healthcare and technology, optimizing service offerings through high-touch interactions.

Qualifications:

  1. Master of Social Work (MSW) or Public Health; or equivalent work experience
  2. Licensed Clinical Social Worker (LCSW) (preferred)
  3. Fluency in Spanish is a major plus
  4. Minimum 3 years of professional social work experience
  5. Minimum of 1-2 year(s) of leadership and team management skills
  6. At least 3 years of acute care experience; knowledge of systems--financial, insurance, advocacy, legal; knowledge of community resources
  7. Deep knowledge of social services benefits, social determinants of health (SDoH), mental health, & complex case coordination
  8. Experience in methods application, casework, supervision, training, and team development and coaching
  9. Proficiency in leveraging technology for healthcare solutions is a plus
  10. Collaborative, solution-focused, team player with a problem-solving orientation and a flexible and positive attitude
  11. Passionate about helping others by solving health equity disparities in the US
