Nurse Practitioner Program Description:

Care Partners Medical Group is a partner with PACE to redefine senior care. Each participant works with a team of medical experts to develop a care plan that is personalized and regularly updated to meet their evolving medical and social needs. Senior care is not ‘one size fits all,’ which is why each participant has a customized PACE experience to meet their individual needs.

Care Partners Medical Group San Bernardino PACE opened in 2014 and serves approximately 750 seniors in the Inland Empire. The center features a winding hallway that includes historic California art. The center also has a clinical exercise program and a program for participants to learn more about the Internet and social media so they can stay digitally connected to friends and family. The outdoor patio is a participant favorite, hosting activities like morning Tai Chi, tea parties and afternoon gardening. Other popular events are karaoke and the annual three-day luau, which includes a Hawaiian barbeque, live Polynesian dancers, limbo, and more.

Nurse Practitioner Essential Functions and Work Responsibilities:

Accommodation can be provided for individuals with disabilities.

  • Performs physical examination and orders and interprets diagnostic tests.
  • Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed.
  • Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition. Provides alternative methods of therapy to participants as appropriate.
  • Educates staff, participants and caregivers regarding healthcare issues and treatments.
  • Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care needs of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings.
  • Provides emotional and spiritual support to participants.
  • Provides management of medical emergencies as needs dictate.
  • Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records.

Qualifications:

  • Current active California Nurse Practitioner license
  • Personal transportation, valid state issued driver’s license, good driving record and auto insurance as required by law.
  • A minimum of five years’ experience as an RN with extensive experience in elderly care
  • A minimum of one year experience as a nurse practitioner in a geriatric setting
  • A minimum of one year working with the frail or elderly is required.
  • A minimum of one year experience as a nurse practitioner in a geriatric setting and full prescriptive authority preferred
  • Current active DEA license

Knowledge, Skills, and Abilities Necessary:

Technical Skills:

  • Must be computer proficient and possess experience with Microsoft Word, Excel, and Outlook.
  • Possess exceptional English written and verbal communication skills, including accurate grammar and business correspondence knowledge.

  • Must be able to quickly learn specific software and new applications.
  • Even-tempered and able to balance multiple tasks in accordance with changing deadlines and priorities in a fast-paced environment.

Analytical Skills:

  • Able to analyze data and statistics and draw reasonable conclusions and compile accurate report.
  • Ability to define problems, collects data, establish facts, and draw valid conclusions.

Interpersonal Skills:

  • Able to establish and maintain cooperative and positive working relationships.
  • Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.
