Job Overview:
The Associate Clinical Social Worker - Bilingual Mandarin or Cantonese, ACSW, will assist the Licensed Clinical Social Worker (LCSW) in providing assessment, diagnosis, treatment, crisis intervention, case management for members with mild to moderate behavioral health needs.

Essential Job Functions:

• Provides assistance to the LCSW in the full integration of primary care and Behavioral health care.
• Assists with warm handoffs between primary care providers and behavioral health clinicians.
• Reviews medical providers’ referral orders for behavioral health services through the Electronic Health Record Report Manager and schedule appointments for referred patients.
• Works with LCSW to create a structured system for follow-up phone calls to patients who are no-shows.
• Performs follow up calls to improve patient self-management and adherence to treatment plans.
• Tracks and follows up on patients who have no-show appointments.
• Performs other job duties as required by the specialty or by manager/supervisor.

Some Responsibilities:
- Conduct comprehensive assessments of patients' social and emotional needs
- Develop individualized care plans based on assessment findings
- Provide mental health counseling and therapy to individuals and families
- Utilize evidence-based practices such as motivational interviewing and psychotherapy
- Collaborate with interdisciplinary teams to coordinate care and services
- Advocate for patients' rights and access to resources
- Monitor and evaluate patient progress and adjust treatment plans as needed
- Provide crisis intervention and support during emergencies or challenging situations
- Maintain accurate and up-to-date documentation of patient interactions

Qualifications:

• Bilingual Cantonese or Mandarin Required
• Holds a MSW from an accredited school of social work, ASW required.
• Experience with one-on-one patient health coaching and/or motivational interviewing.
• Demonstrate leadership skill and effective communication.
• Has great interest in the implementation of integrating primary care and behavioral health care.
• Current documentation of Basic Life Support is required.

Job Type: Full-time

Pay: $49.00 - $57.00 per hour
