Responsible for the delivery of primary health care in a Federally Qualified Health Center

Requirements:

Familiarity with chronic health issues including, but not limited to diabetes, hypertension and asthma, communicable diseases, HIV/AIDS, substance abuse, and mental illness.

Ability to work independently within scope of nursing practice with little supervision.

Ability to communicate and work with diverse populations, including clients, staff, students, volunteers, and the community.

Ability to manage multiple tasks and produce expected results. Good leadership, interpersonal, team-building and communications skills.

Responsibilities:

Triage patients with medical complaints and concerns and refer to provider as appropriate.

Perform intake on patients according to Care Alliance intake procedures.

Perform basic health assessments and screenings.

Perform basic procedures, including venipunctures, injections, EKG’s, and assisting with other procedures.

Work collaboratively with other Care Alliance health professionals to facilitate a team approach to patient management.

Provide education related to specialized health care needs of individuals who are homeless to service providers with whom Care Alliance works.

Facilitates a safe and healthy environment in Care Alliance clinics.

Participate in community outreach, such as immunizations and health education as needed.

Assist with the development of nursing protocols.

Qualifications:

Registered nurse with current Ohio licensure; BSN preferred.

Minimum three years nursing experience, computer literacy required.

Experience as a provider of clinical services; primary care, chronic care, outpatient, community health and/or psychiatric nursing background

Epic EHR and My Chart experience preferred
