The Medical Assistant II is assigned dutiesand responsibilities related to patient care and administrativesupport in a teambased environment while promoting a smooth flow ofpatients through the office.

The Medical Assistant I functionsunder the license of a provider when performing clinical and otherpatient careresponsibilities.

• Effectivelypartners with patients and care management team towards influencingshared goals.
• Effectively navigates and documentspertinent patient information within the Electronic Medical Record(EMR).
• Tracks and monitors patient outcomesrelated to preventative screening and chronic disease managementcontributing valuable insights to interdisciplinary teamdiscussions.
• Appropriately handles patientinteractions identifying actual and potential barriers to adheringto quality interventions involving nursing staff and / or departmentleadership as needed.
• Reviews gaps in care anddisease registries to identify opportunities foroutreach.
• Supplies authorized information tothird parties upon request.
• Assists patients incompleting forms associated with individualized careplans.
• Facilitates previsit planning includingpatient reminders instruction and preparation for specificprocedures and / or warm handoffs asneeded.
• Performs other duties as required orassigned.
• Graduateof an accredited Medical Assistant Programrequired.
• Strong knowledge in Microsoft Officeapplications including Word Excel Outlook and PowerPointrequired.
• Excellent interpersonal customerservice and communication skills both written andverbal.
• Excellent time management andorganizational skills.
• Ability to workindependently as well as in a teamenvironment.
• PreferredQualifications :
• Previous experience inoutpatient ambulatory care setting.
• Experiencein supporting clinical quality programs e.g. HEDIS CareCoordination or Chronic DiseaseManagement.
• Ability to participate in roledevelopment and adapt to role changes as theyevolve.
