CAPTAIN Community Human Services is Hiring a Housing Case Manager (full-time) Near Gloversville, NY

Job Title: Housing Case Manager (Full Time, Non-Exempt)

Hiring Rate: $21.00 per hour

Job Summary:

The STEHP Case Manager will deliver services per the Solutions to End Homelessness Program (STEHP) grant requirements for individuals and families experiencing or at risk of homelessness. This position is 30 hours per with some evening work and flexibility needed for client needs and involves traveling to client homes in the Fulton and Montgomery County service area. The case manager will also manage the Foster Youth to Independence caseload of 2 households in Fulton County and the Permanent Supportive Housing caseload of approximately 3 households in Fulton County.

Duties and Responsibilities:

  • Coordinate the Solutions to End Homelessness Program Rapid Rehousing and Homeless Prevention services to the homeless population throughout Fulton and Montgomery Counties. Meet all grant obligations as identified in the contract as follows:
  • Coordinate the Foster Youth to Independence services. Meet all grant obligations as identified in the contract as follows;
  • Coordinate the Permanent Supportive Housing services. Meet all grant obligations as identified in the contract as follows:

o Meet with potential clients to assess eligibility and assist with the application process.

o Process financial payments to landlords per agency and HUD approved procedures

o Provide ongoing case management to include maintenance of case files.

o Complete HMIS case data entries per requirements

o Work closely with landlords and property managers.

o Coordinate services with appropriate agencies for additional services.

  • Meet and provide outreach to community members, landlords, businesses, local service organizations, school district officials and other concerned neighbors within Fulton and Montgomery Counties to promote CAPTAIN CHS and the STEHP program as needed.
  • Attend monthly Coordinated Entry meetings for Fulton and Montgomery Counties and enter eligible clients into the Coordinated Entry List as needed.
  • Collaborate with the Fulton Montgomery Street Outreach team as needed to meet client needs
  • Travel to CAPTAIN offices in Saratoga County as needed for consultation, supervision and agency events.
  • Complete reports for funding sources and agency administration.
  • Ensure all quarterly and annual reports are completed and submitted on time.
  • Complete and document 10 hours of job-related training per year.
  • Perform other related work as assigned.

Skills Required:

  • Ability to work with all ages and income levels and be a dedicated client advocate.
  • Knowledge of community resources.
  • Good Communication, networking and public relations skills.
  • Able to work flexible and irregular hours as needed.
  • Able to multi-task and work in a team environment.
  • Solid working knowledge of Microsoft Office products, social media sites and some web based applications.
  • Familiarity with Bridges Out of Poverty constructs (training provided)
  • Demonstrate a positive image for CAPTAIN CHS.

Qualifications:

  • Bachelor’s (preferred) or Associate’s degree from an accredited college or university in human services, counseling, education or related field.
  • 2 years’ experience in the field of human services preferred.
  • Must possess a valid NYS Drivers License, clear a DMV check through the Agency insurer, and provide evidence of appropriate insurance coverage.
  • Ability to lift up to 50 lbs.

Telecommuting and Remote Work: The primary functions of this position require in-office and in-community services. Occasionally, project assignments could be accomplished remotely with supervisor permission.
