Job Title: Family Peer Advocate Worker (Per Diem, Non-Exempt)
Hiring Rate: $19.00 per hour
Job Summary: A successful Candidate for this position have lived experience with a child/ youth who have developmental disabilities, social/ emotional challenges, addiction, and/ or behavioral health challenges, in their school, home and community. This position requires flexibility, travel, and providing services to families and their children in their home. Family Peer Support Services are provided by an individual who holds a Family Peer Advocate credential.
Family Peer Support Workers collaborate with the Care Manager and family to design appropriate goals to support the parent/caregivers in advocating for their child to live successfully in their homes, school, and within their communities. These services include developing parenting skills, empowering, building support services and connecting with groups within their community.
Duties and Responsibilities:
§ Provide services based on the child’s needs as identified in the Plan of Care and Treatment Plan
§ Coordinate with the child and caregiver on the scheduling services
§ Work with family members to identify goals and objectives of treatment
§ Attend relevant meetings and trainings as required
§ Implement strategies that are structured, planned and organized through various settings in home or out in the community.
§ Assist individuals and families to identify personal strengths and resources for positive change
§ Act as a resource for children and their families
§ Complete written documentation according to agency policy
§ Maintain on going regular communication with the family and Program Manager
Qualifications:
· Must be willing and able to commit and receiving credential as Family Peer Advocate (If already not certified)
· Must have a minimum of a high school education (a high school diploma or G.E.D.),
· Must be at least 18 years of age and have experience working with children and families (preference given to those with experience working with children with special needs)
· Must be cleared by the State Child Abuse Registry; and complete fingerprinting for a criminal history background clearance
· Complete all Mandated trainings
· Must possess a valid and clean NYS Driver’s License and provide evidence of appropriate insurance coverage.
Skills Required:
§ Excellent organizational and interpersonal skills
§ Good Communication skills and proven ability to work with special needs children and their parents.
§ Ability to work collaboratively with a multi-disciplinary team
§ Ability to work with all ages.
§ Able to work independently and meet state and federal mandated deadlines
§ Knowledge of community resources.
§ Positive role model for CAPTAIN CHS and dedicated to working in the community.
§ Computer experience required
Telecommuting and Remote Work:
Generally, work from home is not a viable option as this position requires significant in-person presence. Working from home will be considered in unusual circumstances and only when the Executive Director approves such a request
