Job Summary:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for coordinating patient care, providing support, and ensuring the delivery of quality services to individuals in need. Responsibility:
- Conduct assessments to identify individual needs and develop care plans
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care
- Monitor patient progress and adjust care plans as needed
- Provide counseling and support to individuals facing mental health challenges or addiction
- Maintain accurate and confidential patient records in compliance with HIPAA regulations

Qualifications:
- MHRT-C (Required)
- Bachelor's degree in Social Work, Psychology, Counseling, or related field (preferred)
- Previous experience in behavioral health, managed care, addiction counseling, or related field
- Knowledge of medical terminology and familiarity with acute care settings
- Understanding of HIPAA regulations and patient confidentiality
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment such as hospitals or mental health facilities
