Job Description: The QIDP/Program Coordinator is responsible for overall operations including financial management, monitoring of assigned House Managers. He/She is assigned to 4-6 homes. Travel between homes is required. The QIDP is responsible for creating an atmosphere of support and empowerment both for his/her team and above all the individual concerned. Be a role model to the House Manager, projecting team spirit, and presenting a professional appearance and image

Minimum Education:

  • Bachelor’s Degree in a Human Services
  • Master’s Degree (Added Advantage)

Minimum Experience:

  • 7 years in the human services field
  • 5 years with persons with developmental disabilities
  • 3 years in a supervisory role
  • (Any equivalent combination of experience and training, which provides the required knowledge, skills, and abilities.)

Job Duties and Responsibilities: The QIDP will be assigned to a number of homes, performing (but not limited to) the following tasks:

  • Train staff on data collection.
  • Write and implement the day-to-day programs of the individuals we serve.
  • Prepare court reports before court hearings.
  • Prepare monthly and quarterly reports on individual’s progress, etc.
  • Monitor medication administration (Working closely with nurses, physicians and other professionals in this area)
  • Working closely with House Managers and other staff; monitor individual’s diets, writing daily menus and preparing shopping lists, grocery shopping,
  • Managing individual’s finances for petty cash, outings, clothing, etc
  • supervise delivery of waived services
  • Facilitate all client progress and special needs meetings as requested by the case manager, day program and care providers
  • Develop quarterly and annual reports, such as Provider Annual Assessment and Recommendations, Risk Management Assessment and Plan, Progress Reports and Program Plans within timelines established by company policies
  • Assist in completing necessary referral information for new admissions and ongoing progress reviews
  • Ensure that individual’s information is updated on an ongoing basis
  • Advocate on behalf of individual’s
  • Meet with the QIDP Supervisor to discuss and review assigned caseload. Frequency of contact will be reviewed and revisions will be made as needed
  • Assisting supervisor in scheduling all meetings concerning individuals and attending and contributing in these meetings
  • Promotes a positive self-image of each individual
  • Performs all other duties as assigned by Supervisors

  • Management and supervisory skills
  • Knowledge of local communities and their activities
  • Capable of working responsibly with highly confidential information
  • Must be able to work as part of a team as well as independently
  • Ability to carry out programs with creativity and initiative
  • Must be flexible and organized
  • Maintain good judgment when faced with an unexpected or disturbing turn of events

Job Requirements/Qualifications and Certifications:

  • 25 years or older
  • Bachelor’s degree at the least
  • Driver’s License and Reliable Transportation
  • Proof of Hepatitis B Vaccine
  • PPD/Chest x-ray and Physicals
  • American Red Cross Standard First Aid Certification
  • Cardio-Pulmonary Resuscitation (CPR) Certification
