A Skilled Nursing Facility is seeking a Full Time Social Worker. The ideal candidate will have a Bachelors Degree in Social Work (LMSW Preferred) and experience as a Social Worker in a long term care setting. The Social Worker will be responsible for:
- Initiating discharge planning process at admission
- Running care plan meetings, scheduling them and inviting families
- Admission Agreements and Assessments upon admission
- Setting up discharges for rehab residents from start to finish to include ordering equipment and services at home
- Audits
- HCP, POA, MOLST compliance
- Admission Assessments and Care Plans
- MDS experience is required
- Other duties as assigned
