Position Description:
Wellness Specialist Community Health and Engagement (CHE)The position is full-time with benefits and depends on grant funds, which expire on October 14, 2022. There will be multiple concerted efforts to seek additional funding in the interim.

POSITION SUMMARY: The Wellness Specialist ensures that CHE clients and participants in Project Roomkey (PRK), especially those without supportive services, can access or continue to access health and wellness services such as medical, dental, and behavioral health in Monterey County. Also, this position will provide a range of care coordination services to clients exiting out of PRK and transitioning into stable housing situations so that they can retain access to health and wellness services up to 12 months after moving into housing. This position is under the direct supervision of the Wellness Supervisor.
ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to, the following:
  • Respond to referrals submitted by CHE case managers or PRK coordinators within 24 hours or less, depending on the participant's needs and circumstances.
  • Help clients, in consultation with their medical providers, to develop and adhere to a care management plan.
  • Support clients in completing referrals (e.g., Primary Care Physician, Behavioral Health, dental, and other specialty services).
  • Help clients establish a medical home (i.e., primary care provider) for those who do not already have one.
  • Assist clients with scheduling and attending healthcare appointments, including delivering appointment reminders and accompanying clients, either in-person or via telehealth.
  • Work with PRK Coordinator to arrange transportation to healthcare appointments.
  • Assist clients with completion and renewal and eligibility benefits to help clients enroll and remain enrolled in health coverage; Provide information on health and community resources to clients.
  • Provide participants with appropriate coping tools and strategies to address individual events to respond appropriately and supports their housing goals.
  • Assist participants in recognizing the triggers that could impact their decision-making which may prohibit them from reaching their housing goals.
  • Assist participants in understanding their emotions and learn techniques to normalize behaviors and responses.
  • Use Trauma-Informed Care to support the participants and reduce their barriers towards housing goals.
  • Use conflict resolutions and relationship-building techniques to manage situations impacting participants' housing goals.
  • Teach emotional regulation through cognitive-behavioral strategies to de-escalate difficult situations.
  • Assist participants in developing a wellness plan that includes warning signs, emergency support contacts, emergency service agency contact, and coping strategies to de-escalate the source of distress. Reassess the wellness plan periodically and follow up with recommendations for changes.
  • Facilitate groups and life skills classes to optimize participants' health and wellness.
  • Connect and coordinate with service provides who will further assist the participants with their housing needs.
  • Use motivational interviewing techniques to help participants find the motivation to change the behaviors to increase their chance of meeting their housing goals.
  • Work collaboratively with participants to develop their own housing goals.
  • Work with the participants will be done in an empathic, culturally sensitive manner.

This position requires working with students, staff, and faculty. The health and safety of our students, faculty, and staff remain our highest priority. Knowledge, Skills and Abilities:
  • Must have a working understanding of Psychiatric Disorders and expertise and ability to implement the following evidence-based models: Harm Reduction, Psychosocial Rehabilitation, Strength-based Case Management, and Motivational Interviewing;
  • Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups while providing direct services;
  • Intervene effectively in crises with de-escalation techniques;
  • Ability to adapt to unexpected events, delays, or changes or in the work environment; manage demands and prioritize tasks;
  • Strong knowledge of Microsoft applications, social media, and internet software; ability to synthesize complex data and use intuition and experience to complement existing information;
  • Consistently arrive on time and work as scheduled; work evenings and Saturday shifts as needed;
  • Excellent verbal and written communication skills; ability to speak effectively before clients, staff; visitors, students, and volunteers; write correspondence and routine reports;
  • Good problem-solving skills; ability to formulate alternative solutions to problems when necessary and transfer learned experiences from past events to new events with similar situations;
  • Ability to complete duties with accuracy and articulate ideas to improve and promote quality in work. Constructively apply feedback to enhance performance;
  • Ability to treat clients with kindness while being able to enforce rules;
  • Ability to work in an environment where clients are sometimes intoxicated, inconsistent in their behavior, and wary of service technicians;
  • Ability to approach clients with an awareness and acceptance of the gender, cultural, racial, social, sexual, and economic contexts within their lives;
  • Capable of dealing with ambiguous situations (ethical dilemmas); ability to seek consultation and feedback from supervisors and colleagues to advance skills in providing services;
  • Ability to regularly stand and walk; actively listen and talk; frequently lift to 25 pounds, push, pull, carry, reach, and grasp, occasionally stoop and kneel; see 20 feet, with or without corrective lenses.
Minimum Qualifications:
  • BSW/BA or Masters in Social Work or Psychology from an accredited university.
  • At least two years of experience working with individuals with serious mental illness, substance use disorder, or co-occurring disorders.

Compensation and Benefits:
The University Corporation is committed to providing our employees with a comprehensive program that rewards efforts that are appreciated by your colleagues, students, and the customers we serve. The hourly rate for this position is $30. This is a non-exempt position and as such is eligible for the overtime provisions of the Fair Labor Standards Act.
