Position Description:

POSITION SUMMARY:The Social Worker position is responsible for planning, coordinating, and implementing care coordination and helping participants foster their capacities and coping mechanisms in order to improve and better their daily lives and experiences, cope with difficult times, and overcome dependencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to, the following:
  • Organize and facilitate psychoeducation groups and life skills classes to help participants to overcome dependencies, recover from illness, and adjust to life.
  • Refer participants to community resources to assist in recovery from mental, substance use, or physical illness and to provide access to services such as domestic violence counseling and parenting classes.
  • Help clients, in consultation with their medical providers, to develop and adhere to a care management plan.
  • Support clients in completing referrals (e.g., Primary Care Physician, Behavioral Health, dental, and other specialty services).
  • Help clients establish a medical home (i.e., primary care provider) for those who do not already have one.
  • Assist clients with scheduling and attending healthcare appointments, including delivering appointment reminders and accompanying clients, either in-person or via telehealth.
  • Assist clients with completion and renewal and eligibility benefits to help clients enroll and remain enrolled in health coverage; Provide information on health and community resources to clients.
  • Provide participants with appropriate coping tools and strategies to address individual events to respond appropriately and supports their housing goals.
  • Assist participants in recognizing the triggers that could impact their decision-making which may prohibit them from reaching their housing goals.
  • Use Trauma-Informed Care and Motivational Interviewing techniques to help participants find the motivation to change their behaviors to increase their chance of attaining their health and wellness goals.
  • Use conflict resolutions and relationship-building techniques to manage situations impacting participants' health and wellness goals.
  • Teach participants emotional regulation through cognitive-behavioral strategies to de-escalate difficult situations and assist participants in understanding their emotions and learn techniques to normalize behaviors and responses.
  • Assist participants in developing a wellness care plan that includes warning signs, emergency support contacts, emergency service agency contact, and coping strategies to de-escalate the source of distress. Reassess the wellness plan periodically and follow up with recommendations for changes.
  • Connect and coordinate with service providers who will further assist the participants with their housing needs.
  • Monitor, evaluate, and record client progress according to measurable treatment goals described in the wellness care plan.
  • Work with the participants will be done in an empathic, culturally sensitive manner.
  • Completes home visits as required.

Knowledge, Skills and Abilities:
  • Must have a working knowledge of Psychiatric Disorders and expertise and ability to implement the following evidence-based models: Harm Reduction, Psychosocial Rehabilitation, Strength-based Case Management, and Motivational Interviewing;
  • Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups while providing direct services;
  • Intervene effectively in crises with de-escalation techniques;
  • Ability to adapt to unexpected events, delays, or changes or in the work environment; manage demands and prioritize tasks;
  • Strong knowledge of Microsoft applications, social media, and internet software; ability to synthesize complex data and use intuition and experience to complement existing information;
  • Consistently arrive on time and work as scheduled; work evenings and Saturday shifts as needed;
  • Excellent verbal and written communication skills; ability to speak effectively before clients, staff; visitors, students, and volunteers; write correspondence and routine reports;
  • Good problem-solving skills; ability to formulate alternative solutions to problems when necessary and transfer learned experiences from past events to new events with similar situations;
  • Ability to complete duties with accuracy and articulate ideas to improve and promote quality in work. Constructively apply feedback to enhance performance;
  • Ability to treat clients with kindness while being able to enforce rules;
  • Ability to work in an environment where clients are sometimes intoxicated, inconsistent in their behavior, and wary of service technicians;
  • Ability to approach clients with an awareness and acceptance of the gender, cultural, racial, social, sexual, and economic contexts within their lives;
  • Capable of dealing with ambiguous situations (ethical dilemmas); ability to seek consultation and feedback from supervisors and colleagues to advance skills in providing services;
  • Ability to regularly stand and walk; actively listen and talk; frequently lift to 25 pounds, push, pull, carry, reach, and grasp, occasionally stoop and kneel; see 20 feet, with or without corrective lenses.
Minimum Qualifications:
  • Equivalent to a Masters Degree in Social Work (relevant professional-level work experience may be substituted for the required degree).
  • 5 years of social work experience working with clients facing the challenges of homelessness, mental illness, substance abuse, poverty, and/or other significant barriers
  • Case management experience, and community care coordination and/or mental health case management experience are highly desired
  • Familiarity with Safety-Net service programs is preferred
  • Able to work after hours, if needed
  • Demonstrated understanding of and commitment to CSUMBs Vision

The person holding this position is considered a "mandated reporter" under the California Child Abuse and Neglect Reporting Act and is required to comply with the requirements set forth in CSU Executive Order 1083 as a condition of employment.

Compensation and Benefits:

Salary placement is determined by the education, experience, and qualifications the candidate brings to the position, internal equity, and the hiring departments fiscal resources. Classification: Grade 4, Staff. The hourly rate for this position is $30. This is a non-exempt position and as such is eligible for the overtime provisions of the Fair Labor Standards Act.
