Summary:
Café Momentum Nashville seeks an experienced Case Manager to assess, plan, implement, monitor and evaluate actions required to meet interns’ health and human service needs. As a part of the Program Team, this person works closely with the Program Manager and Culinary Team to support programing and catering operations, ensuring interns are properly trained for service. Case Managers report to the Program Manager. They work collaboratively with other team members within the program to ensure the needs of the interns are met by taking a holistic approach. Case Managers also are responsible for cultivating and maintaining relationships with community organizations that provide services and support for interns.
