Job Summary:
Associate Director of Care Management

The Associate Director of Care Management (ACDM) heads the Care Management Department and is responsible for all department activities’ administrative, compliance, business development, and performance management. The ADCM should ensure that all care management functions are conducted in accordance with the guidelines and procedures outlined in the Health Homes’ policy manual, ensuring that all staff within the department receive training that meets these requirements. Additionally, the ADCM operates as the department’s internal Quality Improvement Officer reporting to the Quality Assurance & Improvement (QA&I) Department for all services administered by the Care management team. Services could include support for the agency’s difficult-to-service home care clients. Key roles of the ADCM include:

  • Drive the department’s business development activities focused on building relationships with referral partners and identifying RFP and grant opportunities to expand the services of the Care Management Department.
  • Partner with the marketing and community health workers every week to support the management of new referrals and referral opportunities.
  • Oversee intake screening and onboarding program to ensure that referrals are enrolled on a timely basis for the program’s growth.
  • Supervise the Program Manager, specifically focused on compliance, quality assurance, and management of department services to clients.
  • Provide a backup to the Program Manager and other staff as needed.
  • Monitor department reports and create learning opportunities for the care team to address deficiencies in the comprehensive assessment and reassessment process to ensure that all care staff follow DOH’s quality guidelines and our lead Health Homes.
  • Support Care Coordination staff in providing crisis intervention services, review crisis management plans, and revise Care Plans as appropriate.
  • Review sample service interactions to ensure that the quality services identified in the Care Plan are delivered on time and that the client is satisfied with the services provided.
  • Review all reports of serious incidents, allegations of abuse (including reporting child abuse or neglect), or sensitive situations, and review/sign off on all incident reports per applicable agency and regulatory policies.
  • Facilitate interdisciplinary conversation, coordination, and planning and collaborate with the clinical and service team to manage care coordination resources and target the neediest members for services.
  • Administer performance appraisal for the Program Manager, review and evaluation, and provide feedback for all other staff.
  • Work with the fiscal team weekly to ensure every program meets vital fiscal indicators.
  • Generate at least a bi-weekly report on the department’s KPIs.
  • Participate in the rollout of new programs and optimization initiatives of the agency, including onboarding new customer-facing technologies, annual budget, and other assigned tasks.
  • Bachelor’s in human services, Social Work, Master’s Degree Preferred
  • At least 7 years of Experience in Care Management for Disadvantaged Populations
  • 3 years of Care Management Supervisory Experience

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year
