WHAT YOU CAN EXPECT Clinical Pharmacotherapy Services and Medical Team Collaboration Provides clinical pharmacy services as outlined in approved hospital policies, including but not limited to anticoagulation management, chronic disease state management, and/or providing evidence-based patient care in accordance with a protocol and/or collaborative drug therapy management agreement. Collaborates with allied health disciplines to provide quality patient care. Provides clinical consultation and clarification concerning issues in drug therapy, medication reconciliation, patient care, and transitions of care medication needs. Attend rounds, patient care meetings or other similar activities as warranted. Leadership, Documentation, and Analysis Directs the development, implementation, and maintenance of clinic initiatives. Develops patient-centered clinical pharmacy services in an outpatient setting such as improving services currently offered, leading the deployment of expanded services, and evaluating financial performance and impact of services. Documents pharmacist interventions. Ensures clinical pharmacy services in the assigned area are compliant with all regulatory, professional and departmental standards. Generates reports at the necessary frequency and uploads them to applicable locations. Analyzes data for use by department, including for quality improvement initiatives. Reports adverse drug reactions and medication errors in the event reporting software. Drafts, reviews, and revises medication related policies, procedures, guidelines and protocols. Attends and participates in local and system-wide committees to improve the medication use process. Reviews adverse events related to the medication use process, recommends improvements to the medication use process as needed, and ensures improvements are implemented. Student and Resident Supervision, Staff Education, and Literature Review Directs and precepts students and/or residents. Coordinates and delivers ongoing staff education for clinical pharmacy specialists and residents. Ensures that professional competence of clinical pharmacy staff is maintained through educational opportunities. Reviews literature and publications in area of specialization and recommends improvements to the medication use process. QUALIFICATIONS Required Bachelor's Degree Required Doctorate 1 year PGY1 Pharmacy Residency Preferred 3 years In area of specialization Preferred 1 year PGY-2 Pharmacy residency Preferred Registered Pharmacist (RPh) - . Board Certified in applicable area of specialty or pharmacotherapy (if specialty area not available) - . Required within 24 months of start date or within 24 months of eligibility to test, whichever is later.
