We are currently seeking a Social Worker for Skilled Home Health Care to work in the San Francisco and Marin County areas. You should possess a demonstrated ability to work effectively with patients/families dealing with life-threatening illness and death, along with a strong working knowledge of social work practices.
Essential Duties & Responsibilities include (but not limited to):

• Evaluates the need for medical social casework services to patients in their place of residence:
• Participates in ongoing interdisciplinary assessment of the patient.
• Assesses social and emotional factors relating to the patient's condition to estimate the capacity of the patient and family to cope with problems of daily living.
• Participates in ongoing interdisciplinary assessment of the patient.

Education/Experience:

• Master’s degree in social work from a school accredited by the Council on Social Work Education or Master’s degree in mental health discipline.
• Minimum one-year’s social work experience in a health care setting.

Certificates, Licenses, Registrations:

• Current Licensed Clinical Social Work (L.C.S.W.) certification in the State of California may be required for some positions.
• Certification to provide clinical supervision as required by the Board of Behavioral Science Examiners may be required for some positions.
• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Pay range:
$42.00 - $52.00 hourly
