As a Housing Liaison, you'll practice Housing First, harm reduction, and low-barrier service models when working with individuals or households. You will attend Coordinated Entry/Case Conferencing meetings within the local Continuum of Care, outreach to households experiencing a housing crisis or homelessness, and refer and/or transport individuals to establish them in necessary services. Key Responsibilities:

  • Innovative Service Models: Practice Housing First, harm reduction, and low-barrier service models, prioritizing housing stability and individual needs.

  • Community Collaboration: Attend Coordinated Entry/Case Conferencing meetings within the local Continuum of Care, fostering collaboration and coordinated support for individuals experiencing homelessness.

  • Outreach and Engagement: Outreach to households experiencing housing crises or homelessness, providing compassionate support and guidance.

  • Referral and Support: Refer and/or transport individuals to establish them in necessary services, ensuring access to essential resources and support networks.

  • Landlord Engagement: Build and strengthen relationships with local landlords, providing education on community programs and referrals, promoting housing opportunities for individuals experiencing homelessness.

  • Continuum of Care Coordination: Connect with referrals from Continuum of Care programs to assist individuals with housing stability, facilitating transitions to permanent housing options.

  • Benefits Assistance: Utilize SOAR training to assist individuals in applying for benefits, enhancing financial stability and access to resources.

  • Transition Support: Assist households in identifying readiness to transition to other permanent housing options, providing guidance and support throughout the process.

  • Training and Development: Participate in state-wide Housing Liaison training and calls, staying informed about best practices and emerging trends in housing support.

  • Data Management: Complete timely data collection and entry as required by DMH, ensuring accurate and up-to-date records for reporting and evaluation purposes.

  • Community Resource Navigation: Become knowledgeable about all resources available within the community, serving as a valuable resource to case managers, clients, and agency administration regarding housing and local/state resources.

  • Identification Assistance: Assist eligible households in obtaining identification documents required for housing or employment, overcoming barriers to access and stability.

  • Reengagement and Prevention: Reengage active or former participants disconnected from services or at risk of returning to homelessness, providing proactive support and intervention.

  • Resource Connection: Help households identify and connect with natural resources to prevent or end homelessness, promoting self-sufficiency and stability.

  • Quality Standards: Understand HUD Quality Standards inspections, ensuring compliance and quality assurance in housing placements.

Knowledge, Skills, and Abilities:

  • Exceptional knowledge and skill in supportive housing.

  • Ability to effectively communicate with diverse individuals.

  • Enjoy developing new relationships and working with others within the community.

  • Ability to work with information in a confidential manner.

  • Competent skills working with Microsoft Office applications.

Experience and Education Qualifications:

  • A Bachelor's in Social Work, Psychology, Sociology, or other relevant case manager experience in housing; OR,

  • Associate of Applied Science in Behavioral Health Support from an approved institution eligible to provide community support services; OR,

  • Qualified to provide Community Support as per DMH qualifications guidelines (CS 101 Qualifications dmh.mo.gov).

  • None
