Buckeye Home Health Care of Cincinnati is seeking an LPN who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients.
- Provide care where ever the patient calls home.
- Carry out the plan of care as directed by the patient's physician.
- Work with the physician for medication needs and changes to the plan of care.
- Ability to complete documentation within an EMR System.
- Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
- When appropriate, teach and counsel family members about care and process for disease/injuries.
- Attend orientation, meetings and CEUs
