Brookstone Manor Assisted Living is hiring for a Full time Mental Health Leader position. The Mental Health Leader is responsible for developing and enhancing residents’ psychiatric stability, social competencies, personal and emotional adjustment, and independent and community skills. Duties include but are not limited to:
Instruct, assist, and support residents with medication education & monitoring, basic social and living skills, household management, and successfully transition into & maintaining stability within the site.
Assist with identifying and correcting problem areas and/or the improvement of services.
Coordinate with onsite team members to ensure that services are adequately maintained to meet the needs of residents.
Hold authority to address team members of any wrong doing that needs to be addressed during their shift. This will consist of verbal coaching and documenting the incident for further follow-up by the DOHS or Executive Director.
Communicate with all team members of resident concerns and provide tools to accurately address them.
Assist with new resident transition/acclimation to the site.
Assist with resident care conferences
Daily communication and collaboration with site leadership team
Promote, assist, and facilitate in collaboration with the Activities Coordinator
Accompany residents on scheduled outings (ex. Meals, fishing, etc.)
Provide 1:1 support for residents with scheduled meetings weekly
Train team members on different mental health diagnoses and give them the skills to succeed with the residents
Filling out State and County paperwork as necessary
Skills/ Qualifications:
Must be twenty-one (21) years of age
Current drivers license
Clearance of a Minnesota Department of Human Services background study
Interpersonal relations
Planning & prioritization
Teamwork
Minimal of 1 year of formal experience with mental health care and challenges
Timely completion of ongoing online training requirements
Able to make independent and adaptable decisions when action is warranted
Able to communicate information concerning residents condition
Follow policies and procedures
Willingness to work a flexible schedule
Ability to motivate team members in a positive manner, seeking excellence in care
Ability to actively participate in and easily facilitate group meetings
Ability to maintain composure in stressful situations
Working environment:
Exposure to chemical and respiratory hazards
Minimal exposure to blood-borne pathogens and infectious diseases
Minimal sitting
Moderate noise level
Physical requirements:
Able to lift, push, pull and carry a minimum weight of approximately 40lbs
Able to sit, stand, or walk for long periods of time
Able to kneel, squat, crouch, bend and crawl
Able to work In confined spaces
Able to read, write and communicate in English
Able to tolerate exposure to dust and cleaning chemicals
Good health and demonstrate emotional ability
Able to assist in the evacuation of residents
May include both indoor and outdoor (transportation assistance with/for resident activities)
Possible schedule adjustments for evening or weekend special activities as necessary
