Northland Counseling Center is Hiring an Adult Mental Health Case Manager Near Grand Rapids, MN
Adult mental health targeted case management (AMH-TCM).
* Helps an adult with serious and persistent mental illness (SPMI) gain access to medical, social, educational, vocational and other necessary services connected to the person’s mental health needs.
* Targeted case management (TCM) services include developing an individual community support plan (ICSP). It also includes referring and linking the person to mental health and other services while ensuring coordination and monitoring of the delivery of services. A case manager provides support and advocacy for a client.
Education Requirement:
A Bachelor's degree in the human services field and qualifies as a Mental Health Practitioner. Good communication, computer, organizational skills. The ability and desire to work with in a multidisciplinary team setting required.
Job Types: Full-time, Part-time
Pay: $21,250.00 - $45,000.00 per year
