BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, we feed, shelter, teach, and support our neighbors to build a stronger community. We have 60 locations throughout the borough providing a variety of programs that offer assistance in the areas of family, children and youth support, educational services, elder care, homelessness, mental health, and workforce development. BronxWorks is seeking a Tenant Services Coordinator/Case Manager to join a new, diverse and enthusiastic multidisciplinary team. Bronx Point provides permanent housing for 82 formerly homeless individuals and families. The ideal candidate has excellent engagement skills, experience in group facilitation and ideas for new programs to bring to the community. Bilingual in Spanish is preferred, but not required. Work schedule is Mon-Fri 9am-5pm or 8am-4pm. Staff are required to work one Saturday per month from 8am-4pm. Qualifications

  • Associates degree required preferably in the social services (Social Work, Psychology, Sociology, Human Services) or Bachelor’s degree preferred preferably in the social services (Social Work, Psychology, Sociology, Human Services)
  • Proficiency in modem business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment Essential Duties and Responsibilities
  • Conduct on-site outreach and advertising for planned events
  • Conduct home visits to clients as needed and actively engage clients who are absent or decrease their participation level and involvement in the services
  • Develop and maintain connections with internal and external agencies and companies who can provide services and programs to clients
  • Make ongoing assessments on what interests clients have, through satisfaction surveys or feedback from clients
  • Liaise with case management staff to collaborate with them on clients' programming needs and concerns, and share relevant information to the case managers.
  • Maintain records of client interactions for audits
  • Complete reports on a monthly basis, or more frequently as required
  • Maintain familiarity with agency and city resources available to clients
  • Document progress notes for all encounters and services provided to clients
  • Participate in regular supervision with supervisor
  • Perform additional duties as assigned

Program Specific Responsibilities

  • Creating and implementing a Needs Assessment for the clients to target gaps in care, interests and needs of the clients
  • Coordinating and facilitating a variety of activities, groups and events that cater to all ages of clients, including children, teenagers and adults
  • Provide crisis interventions and manage emergency situations appropriately
  • Maintain and ensure accuracy of records and documentation of all client interactions, group sessions and events
  • Collaborate with CQI staff, building management, case management and clinical staff to coordinate and run various events, groups, and on and off-site activities

Skills/Abilities

  • Strong writing and oral communication skills
  • Strong presentation skills and ability to represent the agency at functions and meetings
  • Ability to collect and analyze client demographics and outcome
  • Excellent organizational and teambuilding skills
  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of I mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
