BROADSTEP- Wisconsin, Inc . is currently recruiting for a Full-Time Case Manager to work within our residential based programming. The Case Manager provides case management on an as needed basis to all assigned clients in our various Residential facilities (CBRF's). The Case Manager will follow standard Case Management practices, including conducting assessments, developing and adhering to case plans, facilitating and/or participating in staffing’s, completing case notes, completing quarterly reports, and completing staffing reports.
Requirements
Bachelor's degree or Associates Degree in social service field, and one to one year of related experience preferred
Possess a valid WI driver's license & Insurance
