This position is full-time; Monday-Friday with no nights, weekends, on-call or holiday hours! Please note, this is a dynamic role in which the RN Case Manager goes in-field to meet with patients based on their needs. You would be in-field for a majority of the time, with maybe 20% or less of your time spent in-office. The RN Case Manager should be prepared to meet with patients in public settings or in-home as needed.
- Provide Nurse Case Management for men who have complex medical needs by identifying patient/family health needs, and then developing, implementing, and monitoring health care plans to meet patient/family health needs.
- Provide education to patients on HIV disease, other health conditions, medical care issues, lab results and adherence strategies to prescribed medications.
- Maintain knowledge and skills relevant to HIV/AIDS, with emphasis on issues specific to men living with HIV, through self-study and through participation in trainings, workshops, and in-services.
- Maintain accurate, up to date patient records necessary for patient and program monitoring. This includes documentation in Epic, the electronic medical record for clinic patients.
- All other duties as assigned.
- Licensed Registered Nurse in Wisconsin
- CPR & BLS Certification
- Valid Missouri Driver License, insurable driving record, and access to a reliable insured vehicle.
- Willingness to travel is required; ability to be flexible in hours, including some evening hours.
- Knowledge of and sensitivity to the HIV/AIDS patient population, and/or an eagerness to learn about this community.
