Broadstep, Inc. is hiring a Case Manager to provide assistance to our programs and staff in Milwaukee, WI.
The Residential Case Manager will report to the Residential Clinical Coordinator and will be based out of Milwaukee, WI. The Case Managers will oversee families, community case manager, probation officers & Social Workers. This will ensure that evidence-based practices are utilized to ensure a comprehensive therapeutic program is provided for the individuals served. The Residential Case Manager will also ensure that state and agency regulations are adhered to, as well as best practice for providing clinical supervision to all families and clients to make sure their needs are met.
Summary:
We are looking for a process minded, data driven leader to join our team of Case Managers in our residential homes. In this role the Case manager is ultimately responsible for the oversight of appropriate and effective implementation of the methodologies defined in the ISP. Case Managers will provide information to the team/front-line staff regarding progress and course of action.

Key Accountabilities & Responsibilities
•Creating individual service plans and discharge summaries with persistent mentally ill individuals
•Have knowledge of crisis intervention and de-escalation
•File commitment paperwork, document client interactions, and provide communication appropriately with all facets of client care team
Serve as a liaison between families, community case manager, probation officers & Social Workers
•Organize and conduct all recreational group activities, medical, dental and prescriber visits
•Effectively facilitate meetings in a team setting, with family/guardians, probation officers, community case managers other consumers and clinical social workers.
Qualifications
•Possess a Bachelor’s degree in Human Services / Social Work or related field. (Master’s preferred)
•Have a valid WI driver’s license & vehicle while being insurable under company policy
•Excellent communication, presentation and facilitation skills required
•Must be able to exercise sound mature/judgment
•AODA certification preferred
