We are currently seeking a skilled and dedicated LPN Homecare Visit Nurse to join our team. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients by providing comprehensive nursing care during home visits. Schedule Monday thru Friday 8:30AM-5PM

Responsibilities:

• Conduct home visits to assess patients' health status and provide nursing care as prescribed by the healthcare team.
• Administer medications, treatments, and procedures according to established protocols and physician orders.
• Monitor patients' vital signs, symptoms, and progress, and report any changes or concerns to the appropriate healthcare professionals.
• Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health outcomes.
• Collaborate with other members of the healthcare team, including physicians, therapists, and social workers, to develop and implement individualized care plans.
• Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record.
• HHA Supervisory visits to monitor care.
• Wound care

Qualifications:

• Current LPN license in the state of Ohio .
• Minimum of 1 years of nursing experience, preferably in a homecare or similar setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a collaborative team.
• Compassionate and patient-centered approach to care.
• Valid driver's license and reliable transportation.
