Responsibilities:
- Provide compassionate and quality care to patients in hospice settings
-knowledgeable on SOC for hospice and Hospice criteria
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and report any changes or concerns to the healthcare team
- Assist with pain management and symptom control
- Educate patients and their families on end-of-life care options and provide emotional support
- Monitor and assess patients' conditions, and report any changes or concerns to DON, MD and IDT
- Maintain accurate and up-to-date patient records
-Collaborate with other healthcare professionals to develop and implement patient care plans
-Coordination of supplies, DME, Medications, consults and follow ups for hospice patient
-Provide hospice nursing care in pain management and symptom interventions
- Provide therapeutic care to patient and family
-Thorough documentation in our EMR system (hospicemd)
-Provide Continuous care if needed
-On call for hospice patients and Patient visits during emergency
Requirements:
- Valid RN license in the state of CA.
- Proven hospice experience in patient care
- Strong knowledge on hospice cases and EMR documentation
- Ability to effectively communicate with patients, families, and healthcare team members.
- Excellent critical thinking and problem-solving skills.
- Ability to work independently as well as part of a team.
- Flexibility to work various shifts, including nights, weekends, and holidays.
Requirements:
- Valid RN license in CA
- New grads are welcome to apply
- Strong knowledge of physiology and medical terminology
- Experience in ostomy care, dermal fillers, triage, specimen collection processing, mammography, ventilator management, administering injections, family planning, and laboratory procedures is highly desirable
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Compassionate and empathetic nature
