Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
Reporting to the Director, Programs, the Assistant Director, Programs, Connect to Care, is responsible for supporting the Program Director in the overall management of the street outreach program. This position provides direct supervision to 4-6 staff. The Assistant Director provides each Social Services Supervisor with regular guidance and consistent support for oversight of day-to-day program operations, ensuring high quality client engagement and exceptional service delivery. The Assistant Director supports the Director by maintaining high-level reports that detail the program performance metrics.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Manage social service delivery including program oversight, staff supervision, and training
Manage 4-6 direct reports
Liaise with external stakeholders including various service partners, government, community, and other agencies to ensure responsive and effective program service delivery
Oversee delivery of services including adding individuals to outreach caseload; ensuring individuals are linked to case management; make referrals for psychiatric assessment, and treatment as appropriate
Ensure effective collaboration with internal stakeholders including other street outreach teams, safe haven and stabilization bed sites, Breaking Ground s Central Intake Unit, Compliance, Research and Evaluation, building staff, etc.
Generate regular and as-needed reports on outcomes, goals, and data tracking
Establish and maintain strong and productive working relationships, with an emphasis on positive and proactive problem-solving
Some after hours on-call required
Perform other duties as assigned
MINIMUM QUALIFICATIONS:
NYS Licensed Masters Social Worker (LMSW) required
Minimum 2 years of supervisory experience
Experience working in 24/7 programs and working with homeless or formerly homeless populations strongly preferred
Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients
Ability to work successfully with a wide range of internal and external stakeholders
Demonstrated success in working in a fast-paced environment with multiple priorities
Ability to delegate and motivate staff to achieve deliverables
Ability to communicate (verbally and written) with diverse populations and stakeholders
Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; f amiliarity with Access-based databases and the ability to learn new programs are required
Valid drivers license with a good driving record, as defined by Breaking Ground is preferred
Bilingual preferred
Compensation: $75,00 0.00 - $82,000.0 0 / Per Annum
