**Job Title:** RN Field Hospice On-Call (After Hours and Weekend) Nurse for all of Bonum Hospice Service Area must be willing to travel during oncall hours

Hourly rate to be oncall is provided with Visit rate per visit

**Reports To:** DPCS

**Job Summary:**

The RN Field Hospice On-Call Nurse is a vital member of our hospice care team, responsible for providing compassionate and skilled nursing care during after-hours and weekend shifts. This role requires the ability to assess, plan, implement, and evaluate patient care in accordance with the hospice interdisciplinary care plan. The On-Call Nurse ensures continuity of care for hospice patients and families during non-office hours.

**Key Responsibilities:**

1. **Patient Assessment and Care:** Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs during after-hours and weekend shifts. Develop and implement individualized care plans based on assessments.

2. **Medication Management:** Administer medications, including pain management, and provide education and support to patients and families regarding medication management.

3. **Symptom Management:** Provide expert symptom management to enhance patients' comfort and quality of life. Address symptoms such as pain, dyspnea, nausea, and anxiety.

4. **Documentation:** Maintain accurate and up-to-date electronic patient records, documenting all assessments, interventions, and patient/family interactions according to agency policies and regulatory requirements.

5. **Communication:** Collaborate with the interdisciplinary team to ensure effective communication regarding patient status, changes in condition, and care plan adjustments. Communicate effectively with patients and families to provide education and emotional support.

6. **Patient and Family Education:** Educate patients and families on the hospice philosophy, symptom management, and end-of-life care options. Support decision-making and advance care planning discussions.

7. **Emergency Response:** Respond to patient and family calls, assess the situation, and provide guidance or necessary interventions to address immediate needs. Coordinate with on-call providers as needed. Evaluates the needs of patients and if a PRN visit is needed, will provide.

8. **Continuous Learning:** Stay current with hospice regulations, best practices, and evidence-based care. Participate in ongoing education and training to enhance clinical skills.

9. **Compliance:** Ensure compliance with all federal, state, and local regulations governing hospice care and maintain knowledge of agency policies and procedures.

10. **Quality Improvement:** Participate in quality assurance and performance improvement initiatives to enhance the delivery of hospice care.

**Qualifications:**

- Valid and unrestricted Registered Nurse (RN) license in the state of practice.

- Minimum of [Specify Years] years of nursing experience, preferably in hospice, palliative care, or related field.

- Strong clinical assessment and critical thinking skills.

- Excellent communication and interpersonal abilities.
