GPD Case Manager
ABOUT THE ORGANIZATION:
Established in 1970, Boley Centers, Inc., is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County.

JOB SUMMARY:
Boley Centers, Inc., a nationally recognized and accredited behavioral health care agency, has an opening for a GPD (Grant Per Diem) Case Manager.
This position assesses service and treatment needs of assigned Veterans ready to enter permanent supported housing and ensures all needed services are provided to all Veterans assigned to caseload. Assist in securing appropriate, permanent housing placement, ensuring housing is furnished and community supports, and services are in place. Completes appropriate and necessary reports, records, documents, etc. and files in appropriate records.
EDUCATION:
Bachelor's degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and has a minimum of one year of full time or equivalent experience working with adults experiencing serious mental illness; or bachelor's degree and three years full time or equivalent experience working with adults experiencing serious mental illness. Education must be from an accredited school, college, or university.
EXPERIENCE:
Minimum of one to three years’ experience working with serious mental illness required depending upon type of bachelor's degree.
Knowledge of mental illness and symptoms, mental health system and supports, community social services systems.
SKILLS AND ABILITIES:
Good skills and demonstrated abilities in verbal and written communications, coordination/cooperation, follow-through, interpersonal relations, attention to detail, use of basic language and math, following verbal and written procedures and instructions, recording information orally and in writing, internal and external public relations, working independently, verbal and physical crisis intervention techniques, planning, problem solving, negotiating, flexibility to change, decision making, scheduling own and others' work, counseling techniques and evaluation techniques, organizing own and others' work and such others that are usually and customarily considered necessary for satisfactory completion of essential duties.
SPECIAL REQUIREMENTS:
Valid Florida driver's license, own transportation vehicle and vehicle insurance required.
