Currently, we are actively seeking two full-time Nurse Practitioners, offering flexible working schedules tailored to meet your availability. Our focus is on serving patients across Maricopa County, encompassing both the West Valley and East Valley regions.
Overview:
As a Nurse Practitioner at BluPeak Health, you will play a pivotal role in providing consultation in hospice or palliative care, emphasizing symptom management and supportive care to meet the diverse needs of our patients. Collaborating closely with our Hospice Medical Director and other interdisciplinary team members, you'll ensure comprehensive care delivery and optimal patient outcomes.
Key Responsibilities:
· Offer medical support and coordination to palliative and hospice care patients and their families.
· Provide expertise in pain and symptom management, conduct medication reviews, and assess terminal prognosis for patients, families, and staff.
· Order appropriate therapies and tests as necessary, ensuring adherence to established hospice and palliative care guidelines.
· Conduct patient visits in various settings, including assisted living facilities, hospitals, and other locations.
· Collaborate with palliative care physician and keep primary care physicians informed of services provided.
· Perform face-to-face encounters with hospice patients to identify and monitor existing or potential health issues and assess the appropriateness of hospice recertification.
· Attend interdisciplinary team meetings and engage with other team members as required.
· Maintain accurate patient records, charts, and documents to support effective medical practice and ensure reimbursement for services rendered.
· Adhere to applicable laws and regulations regarding collaborative agreements.
· Cultivate strong relationships with referring physicians and care teams, fostering collaboration centered on patient care excellence.
Qualifications:
· Advanced Practice Nurse license in Arizona.
· Previous experience in hospice or hospice-based palliative care programs preferred, though not mandatory.
· Current CPR certification.
· Strong interpersonal, verbal, and written communication skills essential for fulfilling job responsibilities.
· Demonstrated commitment to outstanding patient care, along with high motivation and a desire to contribute to practice growth.
· Valid driver’s license
