Position Overview:

Join Bluestone as an Care Coordination Referral Specialist where you will work remotely supporting Bluestone Care Coordinators who work with either MSHO/MSC (ages 65 ) members or SNBC (ages 18-64, certified disabled) members. As a delegate of Managed Care Organizations, Bluestone’s Care Coordination-led and data driven model of care aims to increase medical support, reduce identified gaps in care and improve overall health and safety of our members. This is a great opportunity to work with a multi-disciplinary healthcare team while gaining valuable experience in the healthcare field. This role requires excellent communication, organization of multiple spreadsheets, and strong problem-solving skills. This position is home-based and may require minimal travel.

Responsibilities:

• Management of work queues related to appointment scheduling, transportation, interpreter requests,and durable medical equipment
• Telephonic contact with members
  • Scheduling Care Coordinator visits with members
  • Scheduling medical care visits and transportation
  • Utilizing and scheduling interpreter services
• Assisting with obtaining needed medical equipment and supplies
• Documentation and form completion
• Participation in care coordination meetings
• Other duties as assigned by Operations Manager

Qualifications:

Education/Certification/Experience

• Associate’s degree in Human Services, Public Health Community Service, etc.
• 1 years experience working in a related field (hospital, clinical referral specialist, patient registration,
  authorizations.)

Knowledge/Skills/Abilities

• Ability to work independently with excellent time-management and organizational skills.
• Creative problem solving
• Proven ability to communicate effectively both verbally and in written forms
• Excellent interpersonal and customer service skills
• Ability to deal with change and ambiguity
• Knowledge of health plan benefits and Medical Assistance preferred
• Familiar with medical terminology preferred
• Comfortable navigating electronic medical record systems and working with patient care notes
• Ability to problem solve and meet deadlines
• Computer proficient
• Demonstrated ability to read, write, speak, and understand the English language
