Bloomington Primary care is Hiring a Registered Nurse (RN) Near Bloomington, IL

Summary of Duties: Performs nursing duties vital to the medical practice including: phone triage, orders, injections, assessment, procedures, and treatments. Assists the providers with diagnostic and treatment procedures by way of Evidence Based Guidelines and standing orders.

Essential Functions:

Personal Characteristics/Traits/Skills

The Nurse must be a high achiever, goal oriented, organized and have the ability to set priorities. The Nurse must have interpersonal skills to deal professionally and respectfully with patients, staff, providers, and other health professionals both inside and outside the practice. The Nurse must have the ability to work effectively and tactfully with staff, patients, public and external agencies. The Nurse is instrumental in communicating with and assisting the patients and providers.

o Must possess a pleasant personality, patience, tactfulness, confidence and enthusiasm.

o Must possess organization skills and have the ability to multi-task.

o Must be able to prioritize the daily activities of the position.

o Must be willing to work as a team member.

o Must have the willingness and ability to handle difficult situations.

o Must have excellent communication and coordination among the Care Team.

Operational Tasks

· Provides telephone triage for patient phone calls advising patients as appropriate, relaying provider instructions or scheduling an appointment.

· Delegates assignment of the performance of activities or tasks related to patient care to unlicensed assistive personnel while retaining accountability for the outcome.

· Leadership role model who inspires others to work together in pursuit of a common goal, such as enhanced patient care.

· Administers injections and maintains proper documentation.

· Cleans and sterilizes instruments and equipment using infection control procedures.

· Obtains specimens for lab work, prepares appropriate requisitions, and forwards to laboratory. Forwards orders as appropriate to the lab/x-ray department for lab work, x-rays, etc.

· Proactively assists the Care Team in addressing the needs of the patient population.

· Assists in analyzing performance measurements to identify areas of improvement and works with the Care Team to establish and test methods for quality improvement.

· Performs patient education activities, and distributes appropriate materials or resources.

· Attends and actively participates in Quality Improvement and staff meetings.

· Limits access to Protected Health Information (PHI) as minimum necessary.

· Assists with other Care Team functions as deemed appropriate or determined by management.

· Maintains department orderliness and cleanliness.

· Utilizes organizational resources in a responsible/cost effective manner.

Clinical Support/Health Outcomes

Provides support in the delivery of quality clinical care. Provides supervision and ongoing training and unlicensed assistive personnel members of the Medical Home.

· Triages and delegates patient communiation and other duties as assigned as identified by the practice protocol and within the scope of the nurses licensure.

· Performs various tests/procedures and medication administration independently in accordance with the associate’s licensed scope of care.

· Provides patient education and instructions and ensures patient understanding.

· Assists patients in obtaining additional services ordered by the provider and in setting up referrals, medical assistive devices, etc. for additional care.

· Anticipates the need for and completes all documentation necessary to ensure quality care based on policy and procedure, including vital signs, immunizations/medications, history, procedures and messages.

· Accurately performs and maintains practice site quality control logs in accordance with standards for maintaining CLIA certification.

· Accurately maintains tracking log and communicates to the provider any delays in obtaining results. Assists in the communication of test results by following the appropriate policy and procedure for the site.

· Responsible for accurate specimen collection including patient identification and labeling based on laboratory collection procedures.

· Communicates with pharmacies to order new or refill prescriptions per site protocol.

· Communicates with other physicians or other ancillary staff to ensure continuity of care.

· Works with physician and billing department to accurately code and capture charges.

· Participates in “mock code” drills.

· Complies with infection control standards, OSHA compliant.

· Knows where emergency equipment is located and makes it available to the provider.

· Documents according to policy and procedure in medical record all patient communications.

· Takes an active role in achieving Clinical Integration goals assuring accurate data is obtained and documented. Assists the provider in maintaining registries.

Patient Satisfaction and Service

Provides efficient, high-quality service to patients who arrive for appointments or who telephone or visit in person to request appointments or information on tests and procedures, medication refills, referrals, and other matters. Exudes excellent communication and coordination among the Care Team.

· Consitently demonstrates sincere understanding and empathy in interactions with patients.

· In collaboration with the team, meets or exceeds established patient satisfaction targets.

· Treats patients, families and associates with respect and incorporates cultural differences into interactions/care.

· Utilizes motivational interviewing techniques and patient coaching for patient self-management support.

· Responsible for conveying a professional image.

· Participates in activites to evaluate and improve processes that will contribute to patient, associate and physician satisfaction.

· Creates and maintains an environment that is patient centered as evidenced by availability of same day appointments, future appointments made, referrals made within the group, positive patient comments, and high satisfaction scores.

· Functions as clinical support for staff.

· Maintains strict confidentiality in all aspects of patient care and communication in person and via the telephone, HIPAA compliant.

Knowledge/Education Requirements:

· Graduation from accredited school of nursing required.

· Must demonstrate and maintain credentialing and licensing requirements as a Registered Nurse or Licensed Practical Nurse per the Illinois State Board of Nursing.

· Knowledge of professional nursing theory and clinical practice standards.

· Previous medical office experience preferred. Hospital experience will be accepted.

· Current CPR card.

· Requires working knowledge of computer skills and programs in addition to medical software.

Training and Continuing Education (included but not limited to the following):

· Continuing Education Courses/Hours completed as needed for licensing.

· Annual HIPAA and OSHA compliance training.

· Computer Program education.

· Completion of webinars and seminars to field related topics

Physical/Environmental/Working Conditions:

· Works in an office environment.

· Sits, stands, bends, lifts and moves intermittently during working hours.

· Is subject to frequent interruptions.

· Is involved with patients, families, staff and visitors.

· Is subject to hostile and emotionally upset patients.

· Must be able to lift, push, pull and move equipement, supplies, etc.

· Frequent exposure to communicable diseases, body fluids and hazardous substances.

· Flexibility with availability of working hours.
