Responsible for facilitating patients and families in the identification and management of bio-psycho-social, spiritual, financial, development, and cultural issues in the face of acute, chronic, or terminal illness to keep them safe and adequately cared for at home.
- Must have graduated from an accredited school with a Masters in social work.
- Current CPR certification.
- Develop a plan of care for each assigned patient.
