Stanford Sierra Youth & Families is Hiring a Social Worker, Pathways to Permanency (Part-Time) Near Sacramento, CA

*$1,500 Hiring Bonus, Additional $1,500 Spanish Speaking*

QUALIFICATIONS

Education & Experience

  • Master’s degree from an accredited or state approved graduate school in social work or social welfare, marriage, family, and child counseling, child psychology, child development, counseling psychology, social psychology, clinical psychology, educational psychology, education with an emphasis on counseling , or an area that qualifies for licensure as a Licensed Professional Clinical Counselor
  • Meet Community Care Licensing requirements for Social Work Personnel, as follows:
    • 3 semester units or 100 days of field practice/experience in an agency setting at the Master’s Degree level.
    • 9 semester units related to children and families or 18 months experience working with children and families.
    • 3 semester units in working with minority populations, 6 months experience in working with minority populations, or 6 months in-service training in working with minority populations within the first year of employment as a condition of employment.
    • 3 semester units in child welfare, or two years’ experience in public or private child welfare social services setting.
    • At least one year of experience working with youth who have serious behavioral challenges. This can include experience gained in a field placement setting, internship, or other position serving youth with behavioral challenges.

ESSENTIAL JOB FUNCTIONS

All Social Workers

  • Participate in agency 24-hour family support on-call rotation
  • Support general recruitment efforts
  • Case management of youth/families as assigned
  • Support family visitation as needed
  • Support placement efforts for placement of the youth or non-minor dependent in the approved Resource Family home
  • Complete clear, accurate, and timely assessments, case notes, and reports
  • Conduct family evaluations for potential Resource Family homes utilizing the SAFE model
  • Knowledge of family systems issues, the use of active listening and positive behavioral supports
  • Knowledge of Community Care Licensing regulations and seek out consultation as needed
  • Communicate effectively and respectfully with all members of Child and Family Teams
  • Maintain youth and family files as necessary to meet licensing requirements and accreditation standards
  • Assist in social functions, recruitment and retention events, and meetings of the Pathways to Permanency program
  • Participate in utilization, accreditation, and peer review process as assigned
  • Conduct meetings and continuing education and pre-approval trainings with Resource Families
  • Responsible for meeting all outcomes as documented in Employee Development Plan.
  • Seek out clinical information and advice as needed, accepts constructive feedback and reasonable recommendations from manager
  • Perform other duties as necessary for the program/agency, including working in other programs

Position Specific

Focus Area: Permanency

Responsible for preparing children and families for permanency through conducting family evaluations; developing, coordinating, and managing needs and service plans for youth in Resource Family homes; and ensuring all needs of the youth are being met.

  • Provide case management services for youth and families as assigned by the Clinical Program Manager
  • Create and implement needs and services plan, to help maintain stability and functioning of youth and the Resource Family
  • Utilize needs and services plan and collaborate with other community partners and birth family to create a unified treatment team and support the permanency plan
  • Conduct weekly or bi-weekly home visits to monitor youth’s progress and provide support and advocacy services to the Resource Parent, youth, and birth family
  • Provide referrals and resources to youth, non-minor dependents, and families
  • Coordinate visitation schedule with birth family and ensure supervision as ordered by the courts
  • Maintain contact with the youth’s birth parents or other designated relatives, regarding the permanency plan as appropriate
  • Act as liaison with County Social Worker/Probation Officer regarding youth’s progress and placement goals.

Focus Area: Permanency Coordinator

Responsible for nurturing relationships with county partners and Resource Families to create strong connections for placement of youth with Resource Families.

  • Respond to all placement requests and referrals for all offices in a timely and professional manner, in accordance with the policy and procedures
  • Review available children and families and look for possible matches
  • Conduct meetings with families to present available children
  • Collaborate, build, and maintain constructive relationships with internal team members, external team members, placing counties/agencies and community partners
  • Guide families through Search and Referral process
  • Utilize clinical assessments skills/tools and review family evaluation information to request and gather essential information needed to maximize the success of a match between the family and youth
  • Coordinate and facilitate pre-placement visits and disclosure meetings
  • Support placement of referred youth with approved Resource Family homes
  • Assist in opening and successfully transitioning new cases (including managing database)
  • Network and nurture relationships between FFA and County
  • Communicate with Permanency Social Workers regarding updates and changes to family evaluations
  • Support placement of referred youth with approved Resource Family homes

Focus Area: Family Developer

Responsible for the beginning stages of assessing families as a possible resource, as well as ensuring that all components of the application process are complete for Resource Family Approval.

  • Conduct orientations, pre-approval training, and preliminary interviews of potential Resource Family applicants
  • Evaluate and assess and support potential Resource Families and complete associated tasks, forms, documentation review and reports
  • Provide support services and referrals to assigned families including Resource Families in pre-approval and portability processes

Focus Area: Post Adoption

Responsible for supporting youth and families in their permanency. The Post Adoption Social Worker develops, coordinates, and links families with needed support services to promote permanency success and family stabilization. 

  • Provide active support to families and post adoption youth
  • Facilitate support groups and trainings
  • Provide support as needed to Resource Families in the adoption process
  • Assist parents/caregivers in their efforts to articulate and communicate their needs to service providers
  • Case management of youth/families as assigned

Agency Specific

  • Performs all duties in a manner consistent with the principles and values of agency, while adhering to applicable professional codes of ethics, the agency’s policies and procedures, contractor requirements, and regulatory requirements.
  • Model and communicate appropriate positive attitudes toward the agency’s Mission, Vision, and Values
  • Work collaboratively with all agency programs and staff to provide support as needed
  • Utilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkers
  • Participate in on-going training to expand and develop professional skills
  • Perform other duties as necessary for the agency, as assigned

Employment At-Will
Employment at the Agency is terminable at-will, which means that employment may be
terminated at any time, without cause or reason, by either the employee or the Agency. In
addition the Agency may also demote, layoff, transfer or reassign employees at any time at its
sole discretion without cause or reason.

Starting Salary Range: $67,000 - $70,720 DOE
