BFDI is a CARF accredited, licensed, comprehensive human services organization committed to serving all persons, while specializing in evidence-based counseling, treatment, and advocacy for persons of color. Agency programming includes early childhood resources and literacy development, a 3-tiered juvenile justice service array, mental health prevention and treatment, substance use disorder prevention and treatment, boys and young men of color supportive resources, school-based intervention, and trauma-informed care. Our organization is seeking a Full-time Triage Care Coordinator with the corresponding experience requirements:

Salary: $50,00 - $53,000

GENERAL RESPONSIBILITIES

Coordinates care across the spectrum of health services, behavioral health care, social services, housing, educational systems, and employment opportunities to facilitate wellness and recovery of the whole person. Provides home-based and outreach services as needed, with an emphasis on culturally competent services. Makes appropriate referrals to internal and external sources; assists in the development of basic needs and/or needed program services within BFDI, Designated Collaborative Organizations (DCO)s, and/or with other service providers.

DUTIES AND RESPONSIBILITIES

· Provides supportive coordination to include the nine (9) core service area:

1. Crisis Services

2. Treatment Planning

3. Screening, Assessment, Diagnosis & Risk Assessment

4. Outpatient Mental Health & Substance Use Services

5. Targeted Case Management

6. Outpatient Primary Care Screening and Monitoring

7. Community-Based Mental Health Care for Veterans

8. Peer, Family Support & Counselor Services

9. Psychiatric Rehabilitation Services

· Makes initial contact with consumer and/or family within 24 hours of call after case assigned.

· Conducts intake assessment, needs assessment, treatment planning, crisis planning, and reassessment services.

· Screens consumers for eligibility for direct and support services and refer clients to needed services, such as mental health, housing, crisis, and employment assistance, etc.

· Utilizes EHR to record consumers initial evaluation, safety assessment, strengths of person-served, medical care/behavioral health dosages, recovery milestones, and level of functioning changes from BFDI & DCO services from intake until discharge.

· Responsible for assisting with the determination of the appropriate BFDI program.

· Responsible for preparing/assembling all necessary information packets and documents for assigned program.

· Ensures that the consumer and/or family receive and understand all conditions of agreeing to services, recipient rights, and assists with consumer’s signatures on all necessary forms/releases.

· Ensures that all entitlement programs are in place e.g.: medical, dental, social security benefits, etc.

· Maintains regular weekly contact with consumer and/or family until initial appointment occurs within assigned program unit.

· Provides case management follow-up for persons with community-based referrals.

· Provides the initial triage and transfer into BFDI programs.

· Establishes a network of community resource providers.

COMPETENCIES

· Knowledge of developmental disabilities, chronic disease, and social determinants of health.

· Maintain the continuing education requirements of degree(s) and/or licensing body.

· Triage screening skills for adult, youth, and families; ascertain emergent needs of the consumer and/or family.

· Maintain knowledge of community resources to best resolve the individual or family’s emergency needs.

· Demonstrate skills of homicidal/suicidal crisis, assessment, and intervention; document assessment information given by consumer and the intervention services the consumer was engaged in which brought emergency resolution.

· Demonstrated knowledge of agency admission decision criteria.

· Demonstrated knowledge of agency and federal waitlist policy that prohibit program waitlist.

· Knowledge of current program contracts and admission criteria.

· Proficiency in utilizing EHR; including EBO and MHWIN.

· Demonstrated knowledge of the principles of Person-Centered Planning.

· Demonstrated knowledge of the principles of consumerism and second opinion.

· Demonstrated knowledge of limited English proficiency procedures.

· Demonstrate the acquisition of updated community resources.

QUALIFICATIONS

Bachelor’s Degree, Master’s preferred, in Human Service field. A minimum of 2 years working with at-risk youth/families and case management experience. Strong understanding of cultural competency and family-centered services with target population and experience in family-centered. Knowledge of strength-based focused practices, assessment tools, and community resources. Computer literate in both word processing and data base software programs. Willing to work a flexible schedule and maintain 24-hour/7-day week accessibility.
