**Job Description:**
**Duties/Responsibilities:**
Is able to articulate hospice philosophy and provides care accordingly
Functions as a member of an interdisciplinary team that includes the patient and family in facilitating a patient specific plan of care which is driven by the patients end-of-life goals and preferences
Functions, in collaboration with the RN Hospice Case manager, as the eyes and ears of the physician in the home or facility, communicates an accurate assessment of the patients condition to the physician, confers with the RN Case manager and offers guidance on treating end-of-life pain and other symptoms when requested
Provides skilled service that ensures the nursing needs of the patients are met as indentified in the patients initial and comprehensive assessments
Obtains data on the physical, psychological social and spiritual factors that may influence the patients care.
Considers factors related to safety, effectiveness, and cost in delivering patient care. Combines nursing functions and personal care otherwise delivered by a Hospice Aide into one visit to maximize staffing resources and improve patient/facility satisfaction
Maintains up-to-date documentation in the EMR (PTCT) so that problems, plans, actions and goals are accurately and clearly stated and changes can be reflected accurately by all members of the team in the plan of care
Documents in the electronic medical record per agency standards to ensure accurate retrieval of quality indicators
Provides assessment and clear documentation in the EMR to demonstrate patient continues to meet hospice eligibility utilizing appropriate scales (i.e. FAST, ECOG, etc.)
Effectively navigates procurement process of appropriate medications through our national hospice pharmacy
Proactively informs the Clinical Supervisor the Clinical Director of unusual or potential problems
Educates patients, family and contracted caregivers on medications and other care processes to maximize patient comfort
Provides appropriate support at the time of death and offers post mortem care and bereavement support
Demonstrate knowledge of agency policies, State Regulations, The Medicare Conditions of Participation, Joint Commission requirements, Long Term Care Facility regulations and Assisted Living regulations
Knowledgeable of the algorithms for end-of-life symptom management
Functions as part of continuous care team and emergency on-call team as needed

**Education/Experience Required:**
Ability to read, write and speak English
Ability to present in a self-assured, professional manner while functioning independently in a variety of settings
Ability to sustain a positive attitude and person connections despite exposure to continual personal/professional loss
Ability to work effectively within an interdisciplinary team approach to care
Comprehensive knowledge of skills in current end-of-life nursing practice
Minimum of 1-2 years of nursing experience within the past five years
Demonstrated ability to manage all aspects of hospice terminal care, other than care planning and pronouncements
Demonstrated ability to assess and respond to needs of patients and families in varied settings and ability to communicate effectively with people from a variety of backgrounds
Demonstrated customer service skills are essential
Strong computer skills required
Hospice and/or home care experience strongly preferred
**Registration/Certification:**
Current Massachusetts LPN license
Valid Massachusetts drivers license and access to reliable transportation
**Physical/Environmental Requirements**
This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
