Reports to: Director of Nursing
Hours: 20hours per week to include an average of at least 16 hours direct care.

1. The LPN is responsible to exhibit and display the agencies’ mission and basic philosophy of individual validation and empowerment through respectful interaction.
General Statement of Duties: Identify health related needs, monitor medical status and work with residential staff to ensure a safe and healthy environment for individuals living in a community setting. Provide medical related care for individuals living in a supervised community setting. Provide medical related care for individuals living in a supervised residential setting and do so in a manner that promotes growth, independence and dignity for those individuals served.
Responsibilities:
1. Attend to the daily needs of individuals on an individual basis as well as foster increased independence and self-determination.
2. Assist agency nurse in core-comp trainings, annual trainings, medication administration program audits and other trainings that may arise.
3. Review with Nursing Supervisor diagnostic and laboratory data and report abnormal results with appropriate caregivers.
4. Assist with Nursing Supervisor to help provide clinical oversight to REACH’s programs regarding DPH Medication Administration Program.
5. Work with community physicians and program staff to formulate and implement effective medical treatment plans.
6. Ensure safe and proper storage of medications and ensure medications are administered as prescribed.
7. Record and appropriately utilize necessary data, daily progress notes, staff log and complete all other program required documentation.
8. Advise program manager and staff of any medical problems that may hinder an individual’s progress or that may be aggravated by activities at the program.
9. Consult with staff on methods of medical treatment and care.
10. Review medical records and physician orders to ensure program compliance, updating information as needed.
11. Participate in medical appointments
12. Maintain knowledge of ongoing program issues and developments as well as informing supervisor(s) of program issues and concerns.
13. Perform other related work duties as needed or assigned by Nursing Supervisor.
14. Perform duties to reflect agency policies and procedures.
15. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
16. If needed, provide on call support.
17. Utilize Positive Behavior Supports (PBS) practices, to create a thoughtful and supportive environment by ensuring activities, plans, and routines reflect the preferences, needs and goals of individuals.
Requirements
Minimum Required Skills/Qualifications:
· Graduate from a Board Certified nursing program
· Passed the National Council Licensure Examination
· Basic knowledge of Microsoft Office and the internet
· Valid Driver’s License
· Must be willing to authorize a Criminal History Background Check (CORI), and Fingerprinting check through National Criminal Background Check (NCBC) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies
· Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
Language Skills
Must be fluent in English and have the ability to read, and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community. Ability to carry out detailed written or oral instructions.
