- Job Description:
- Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community.
- Social workers facilitate communication and care coordination along a continuum through effective transitional care management.
- Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).
- Required qualifications:
- Master's Degree in Social Work.
- Coursework/Training and All LCSWs must follow the guideline by Los Angeles County Department of
- Mental Health for LPS designation and keep their designation in current status.
- Upon hire: California Clinical Social Worker License
- Within 18 months of hire: Designated as Behavioral Health Clinician to perform 5150 evaluations
- Within 1 year of hire: CPI certification (nonviolent crisis intervention training)
- 2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
- Preferred qualifications:
- Discharge planning experience.
- Candidate Details:
