Job Title: Registered Nurse - Transitional Care UnitJob Summary:We are seeking a compassionate and dedicated Registered Nurse to join our Transitional Care Unit team. The Registered Nurse will be responsible for providing high-quality patient care to individuals transitioning from hospital to home or a long-term care facility.
