Job Title: LPN Primary Care Clinic

Job Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our primary care clinic team. The LPN will be responsible for providing nursing care to patients, collaborating with healthcare providers, and ensuring the delivery of high-quality patient care.

Responsibilities and Duties:

1. Administer medications and treatments as prescribed by healthcare providers.

2. Monitor and assess patient's health status and report any abnormalities to the medical team.

3. Assist in the coordination of patient care, including scheduling appointments and referrals.

4. Educate patients and their families on health conditions and treatment plans.

5. Maintain accurate and detailed medical records.

6. Collaborate with other healthcare professionals to provide comprehensive care.

7. Follow established clinical protocols and guidelines.

8. Adhere to all regulatory requirements and standards of practice.

Qualifications and Skills:

1. Current LPN license in the state of practice.

2. Minimum of 1 year of experience in a clinical setting, preferably in primary care.

3. Strong communication and interpersonal skills.

4. Ability to work effectively in a fast-paced environment.

5. Proficiency in electronic medical records (EMR) systems.

6. BLS certification.

7. Knowledge of nursing principles and practices.
