Job Title: Long Term Care (LTC) Certified Nursing Assistant (CNA)Job Summary:The Long Term Care (LTC) CNA will provide essential care and support to patients residing in long-term care facilities. Under the supervision of licensed nurses, the CNA will assist patients with their daily activities, promote their overall well-being, and ensure a safe and comfortable environment. This role requires a compassionate individual with excellent communication skills and a commitment to delivering high-quality care.Responsibilities and Duties:1. Assist patients with personal hygiene tasks, such as bathing, grooming, and dressing.2. Aid in transferring and positioning patients to maintain their comfort and prevent complications.3. Monitor and record vital signs, including temperature, blood pressure, pulse, and respiration rates.4. Observe and report changes in patients' physical, mental, and emotional conditions to the nursing staff.5. Assist in the provision of specialized care, including wound care, catheter care, and ostomy care.6. Administer medications and document accurately according to established policies and procedures.7. Respond promptly to patients' requests and provide physical and emotional support as needed.8. Assist in transporting patients within the facility and accompany them to medical appointments, if required.9. Encourage patients to participate in social and recreational activities to promote their mental well-being.10. Maintain a clean and organized environment in patients' rooms and common areas.11. Adhere to infection control protocols and maintain a safe environment for both patients and staff.12. Collaborate effectively with interdisciplinary teams to ensure optimal care and positive patient outcomes.13. Provide a comforting presence and support for patients' families and friends when needed.14. Participate in continuing education and training to enhance knowledge and skills in long-term care.15. Follow all state and federal regulations and facility policies while performing duties.Qualifications and Skills:1. High school diploma or equivalent.2. Completion of a state-approved Certified Nursing Assistant (CNA) training program.3. Active CNA certification in the state of employment.4. Prior experience in long-term care or geriatric care settings is preferred.5. Knowledge of basic medical terminology and procedures.6. Strong interpersonal and communication skills.7. Ability to work effectively within a team and follow instructions.8. Compassionate, patient, and empathetic nature.9. Physical stamina and the ability to lift and move patients safely.10. Proficiency in documenting patients' conditions in electronic medical records.11. Basic computer skills and the ability to use medical software systems.12. CPR certification (may be required).13. Flexibility to work day, evening, night, and weekend shifts as per facility requirements.14. Understand and support the importance of resident rights, choices, and independence.15.
