Job Summary:

The Clinical Care Manager RN is responsible for overseeing the delivery of high-quality healthcare services to patients. The Clinical Care Manager RN also provides support and guidance to patients and their families throughout the healthcare process.

Responsibilities and Duties:

• Coordinate and manage patient care plans

• Communicate with healthcare providers to ensure seamless care delivery
• Educate patients and their families about treatment options and health management
• Advocate for patients' needs and preferences

• Evaluate outcomes and make recommendations for improvements in patient care

Qualifications and Skills:

• Valid RN license
• Bachelor's degree in nursing or related field
• Minimum of 2 years of experience in clinical care management

• Ability to prioritize and multitask in a fast-paced environment
• Knowledge of healthcare regulations and standards
• Critical thinking and problem-solving skills
