Berkeley Food Housing Project is Hiring a Healthcare Navigator (SSVF) Near San Francisco, CA

Compensation: $31.73-$33.65 DOE

This position covers 3 SSVF Programs: Contra Costa, San Francisco, and Alameda County based on the needs of the program and clients.

Program Description

The Roads Home Program is a Supportive Services for Veterans Families (SSVF) program designed to serve low-income, primarily homeless Veterans and Veteran families. The program assists Veterans in exiting homelessness to permanent housing and in maintaining a permanent housing placement. Services provided by BFHP include street and venue-based outreach, case management, permanent housing placement, and assistance in obtaining VA and other benefits.

Position Summary

In collaboration with SSVF Program Leadership, the Healthcare Navigator position provides services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. This position provides supervision, training, case management, care coordination, health education, interdisciplinary collaboration, consultation, decision-making, reporting, out-of-the-box solutions to challenging cases, and administrative duties. The Healthcare Navigator will work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team.

Essential Duties and Responsibilities

  • Assist Veterans with gaining entry to VA health care (including mental health care) or community care when Veterans are not eligible for VHA
  • Connecting Veterans to VA health care by working with the VAMC to facilitate enrollment
  • Assisting in gathering documentation and complete paperwork required for enrollment
  • Following up on enrollment progress to ensure that the Veteran is enrolled in VA or community health care services
  • Assist in supporting Veterans in identifying health care needs
  • Work collaboratively with health care teams to facilitate access to care
  • Assist veteran in gaining access to appointments with support
  • Assist veterans in utilizing available services, including preventative health care, including communicating with Veterans and health care teams about appointments
  • Help Veterans identify barriers to adhering to recommended healthcare plans through encouraging communication with health care providers and ensuring coordination of care
  • Assist Veterans in understanding and communicating with providers to make informed decisions about health care
  • Supporting and encouraging Veteran to discuss questions about medication or treatment goals with providers
  • Problem-solve barriers to care (i.e. transportation, childcare)
  • Assist Veteran in identifying barriers to health care goals
  • Help with transportation to health care appointments
  • Provide education or create linkages for Veterans to learn about wellness related topics: Providing pamphlets or other literature on smoking cessation, diabetes management, exercise, etc., Inviting guest speakers to education groups on health-related issues for Veterans, Linking Veterans to support groups or other programs at the VA or in the community to support their health goals
  • Attend and participate in all assigned program and external meetings.
  • Complete the Privacy and Security training and all other assigned staff trainings.
  • Maintain confidentiality regarding clients, personnel and other internal agency affairs.
  • Work within the framework of the BFHP’s Code of Conduct.
  • Be responsible to other staff, clients, BFHP and its stakeholders, both for your actions and your decisions not to act.
  • Conduct business in accordance with the Team Handbook, exercising sound judgment and serving the best interests of BFHP and the community.
  • Commit yourself to treating each community member with respect and dignity.
  • Perform other tasks as assigned by supervisor.

Qualifications, Skills and Abilities

  • Master’s degree in social work, healthcare or related field or the equivalent of 3 years’ experience in the field of health care.
  • Experience working with low income and/or homeless populations.
  • Experience with social services or healthcare settings
  • Effectively communicates with Veterans and health care teams
  • Must be able to establish relationships with Veteran, health care teams, and community service providers
  • Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel)
  • Proficient typing skills.
  • Ability and willingness to work flexible hours to accommodate participants available during the evening or on weekends.
  • Knowledge of social service resource systems and self-help intervention strategies.
  • Independently counsel populations of mental health, substance abuse, homeless individuals
  • Ability to successfully develop relationships utilizing motivational interviewing techniques.
  • Crisis intervention and conflict resolution skills including use of motivational interviewing, harm reduction approach, and trauma-informed care.
  • Thoroughness and accuracy with data collection, entry and quality control in a web-based database.
  • Knowledge of the VA Health Care system strongly preferred.
  • Veteran status, preferred.
  • Proficiency in Microsoft Office, email, internet, and calendaring applications. Experience wit
  • HMIS data entry strongly preferred. Experience with database reporting and extensive data entry.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and crisis intervention skills with a can do and flexible attitude.
  • Ability to work with minimal supervision, multitask, maintain confidentiality, and meet deadlines.
  • Ability to maintain professional conduct, attitude and appearance at all times.

Special Requirements

  • Must have a valid driver’s license and clean DMV record.
  • Must be able to obtain background check clearance.
  • Must obtain and maintain First Aid and CPR certification.

Physical Requirements

While performing the duties of this job, the employee is

  • Regularly required to use hands and fingers to handle, feel, or operate objects, tools or controls, and reach with hands and arms.
  • Frequently required to stand, walk, sit, climb stairs, talk, hear, and see clearly.
  • May be occasionally required to stoop, kneel, or crouch.
  • Must be able to lift or move up to 50 lbs.
