As the Social Services Designee you will play an important role in identifying the best services and resources to ensure enriched lives for our residents. As the Social Services Designee you will develop a team of skilled professionals to carry out the mission of Benedictine and ensure well-being is #1 for our residents.

Responsibilities:

• Relate professionally, positively and cooperatively with community staff at all levels
• Provide support and advocated for resident and/or family. Assist with finding options that best meet the physical and emotional needs of each resident. Gather information to complete assessments per guidelines. Assist in development and implementation of social services care plan. Assist with providing services/interventions for grief, bereavement and end of life issues
• Maintain knowledge of community resources. Assist residents/families with financial and legal matters i.e. POE, Medical Assistance applications, funeral homes etc.
• Responsible for admissions, discharge planning, coordinating care conferences, wait list and room changes pertaining to nursing care. Promotes resident rights and discusses required material with resident and responsible party per regulations
• Designated as the Abuse Prevention Coordinator and provides oversight and consults on any vulnerable adult issues to our continuum of care community
• Coordinate psychological services when indicated

Qualifications:

• Related Coursework in Social Work or Psychology
• Must be able to read, write and communicate in English
• Experience in Hospital or Nursing Home (preferred)
• Experience with the geriatric population
