This position is a part-time, working 2-3 days per week, assisting with discharge planning and social work needs of the facility. Previous experience as a Social Worker in a long-term care and/or acute rehab care facility is preferred. A Bachelors or Masters degree in social work and/or a MI State Social Work license is preferred.
