Indigo Manor is seeking a Case Manager who has the following qualifications:
vMust possess a current, unencumbered, active license to practice as a licensed nurse in this state.
v2 years of experience as a licensed nurse is required. Experience with Case Management preferred.
vCase Management training must be completed within six months of hire.Case Management Certification is preferred.
The case manager will be responsible for the following assigned tasks:
vParticipates in all resource utilization and triple check meetings prior to billing of claims.
vAssists with admission and discharge documentation for residents as needed.
vCompletes all assigned training and skills competency, as determined by the facility assessment and facility training plans.
vPerforms administrative duties as assigned, including but not limited to QAPI activities and participation in department or committee meetings.
The case manager duties and responsibilities include:
vCoordinates and collaborates with insurance companies, the resident and the IDT to ensure that the resident's care needs, and coverage limitations are communicated between the company and the facility.
vFacilitates communication between different healthcare professionals and coverage providers to ensure a cohesive and integrated approach to patient care.
vInquires and shares information about stay expectations and goals
vInform the Resident and the Resident Representative (if applicable) of the opportunity to participate in care plan meetings to discuss billing, insurance coverage, co-pays, care plan development, physician’s orders, diagnoses, dietary needs and preferences, choices, goals and discharge planning
vParticipates in meetings with the interdisciplinary team (IDT) and participates in and documents care plan reviews and revises care plan focuses and goal dates.
vAssists nursing in the maintenance of the resident’s Kardex
vLeads the appeal process
vFacilitates the completion and updating of resident care plans with the IDT to reflect each resident’s current needs and incorporates the resident’s personal and cultural preferences in developing goals of care
vContacts the Resident and the Resident Representative (if applicable) to provide notice regarding coverage changes and upcoming meeting dates and times and maintains records of the contact in the resident’s medical record
vCommunicates the next Care Plan (or Discharge) meeting in conjunction with Assessment schedule and discharge plan to the IDT, Resident and Resident Representative (if applicable)
vInforms the Attending Physician and IDT required but not in attendance of any updates to the plan of care.
