• Graduate of an accredited school of nursing.

• Current NJ licensure as a Registered Nurse

• BSN Preferred

• Physical/TB free from communicable diseases statement Valid state driver’s license and reliable automobile.

• Proof of current automobile insurance.

• Meet with clients and family members to provide the client with an assessment and develop a plan of care in accordance with client’s needs.
• Supervises Home Health Aides on an as needed basis
• Initiates needed referrals to other disciplines and/or community
• Knowledgeable of current federal, state and agency regulations concerning patient
• Adheres to all Agency, payroll, billing and documentation policies

• Attends all required training, in-services, and staff
• Strives to maintain a safe working environment.
• Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers
• Adheres to all policies and procedures of Bedrock at Home NJ, LLC
