The Bedford-Stuyvesant Family Health Center (BSFHC) is a patient-centered, team based multi-specialty practice in central Brooklyn. BSFHC has been providing affordable, comprehensive, outpatient medical and dental services in an outpatient setting for over 40 years. As a Federally Qualified Health Center (FQHC), BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn. We have a Part-time opportunity for a Psychiatric Nurse Practitioner to join our team. The Psychiatric Nurse Practitioner position is crucial to providing quality health care for BSFHC to achieve our Mission.

Scope of Position:

Under the direction of the Psychiatrist, the Psychiatric Nurse Practitioner (PMHNPs) assesses the mental health needs of communities, individuals, families, and groups. They formulate healthcare plans, implement treatment, and evaluate the effectiveness of short- and long-term progress.

Responsibilities and Duties

Duties and Responsibilities may include but not limited to:

• Conduct initial psychiatric evaluation and assessment of pre-existing conditions and current conditions; assess and triage the mental and behavioral health concerns of children, adolescents and adults.
• Provides direct patient care within scope of practice to ensure implementation of plan of care.
• Offer outpatient or home-based care to address sudden changes in a patient’s mental status or chronic conditions
• Serve as consultants for community groups, corporations, healthcare agencies, legislative advocacy groups, and other organizations
• Perform various patient assessments (e.g., physicals, family risk analyses, diagnostic exams, mental health tests)
• Develop holistic, cross-disciplinary treatment plans
• Prescribe medications in compliance with state scope of practice laws
• Collaborate with doctors and make appropriate patient referrals
• Maintain detailed patient records
• Conduct original research and present findings through conferences or scholarly journal articles
• Analyze effectiveness of treatments
• Educate patients, families, and others on challenges in mental health procedures.
• Assists with the supervision of the medical assistants.
• Conducts therapy, medication assessments, and crisis response to new and existing clients.
• Provides therapeutic interventions as necessary including the provision of on-going medication management services.
• Maintains documentation of services including service plans, progress notes, medication informed consent, quarterly treatment plan reviews, and other required information in compliance with agency and third party compliance standards.

Patient Care :

• Coordinate care with referral sources and other involved professionals.
• Maintain regular communication with the involved providers in the primary care practice as appropriate,
• Return phone calls to professionals and patients in a timely manner.
• Encourage patients to participate fully in all aspects of their treatment.
• Educate all patients about ways to facilitate recovery.

Education/Experience Required:

• Current NY State Psychiatric Nurse Practitioner License & Board Certification (PMHNP-BC)
• Versatility, flexibility, and a willingness to work within a fast pace environment

Other Related Skills/Experience:

• Experience working in an outpatient setting
• Experience working individuals in treatment for substance use disorder
• Experience working with children and adolescents
• Familiarity and/or comfort using E-Prescribe systems
